Salary: £20,780-£22,790 per year plus £1000 per year essential car users allowance
Community Connectors is a free service that provides 1:1 support within GP surgeries or community drop-in clinics, creating a personalised plan for patients. We will support people to access specialist services and community activities to enable lifestyle choices that may improve mental health and emotional wellbeing.
The successful candidate will work dynamically and autonomously with patients on a 1:1 basis, either in a GP surgery or within a community outreach clinic. Using basic coaching skills and a Patient Activation Measure tool, you will carry out initial assessments and refer patients to a wide range of agencies to help with issues such as housing, employment, finance, health and wellbeing. You will encourage patients to develop personal strategies to increase confidence, skills and knowledge around self-management of their mental and/or physical health conditions.
You will be expected to engage with a range of partners including GPs, mental health services, wellbeing hubs, employers and family/carers. Candidates will be required to demonstrate their ability to work in a professional, positive and solution-focused manner and to deliver against contractual targets.
This post requires you to be a driver with access to a car for work purposes and may involve working flexible hours outside of 9-5 to fit with GP surgery clinics and client needs.
