Health Navigator

Are you an experienced community outreach worker with a passion for health & wellbeing?
As a valued partner, Sussex Community Development Association will be delivering a large number of NHS Health Checks within the county, and are putting together a first-class team to deliver aspects of the programme. Working with Thrive Tribe, who have recently been appointed to deliver an integrated healthy lifestyle service in East Sussex,
SCDA are looking for a Health Navigator to work in an outreach capacity delivering NHS Health Check provision across some of the most marginalised populations in the county.
As a Health Navigator, you will work in outreach locations across Hastings & Rother delivering health checks, navigating/signposting clients to appropriate health improvement interventions.
We are looking for experienced and dynamic employees who will work in line with our values with a proven track record or desire for genuinely supporting health improvement for people.
Place of work: Based in Eastbourne with travel across Hastings & Rother
Health Navigator: £20,092 - £23,080 pa pro rata
Hours: 1 x 21 hrs pw
All posts are subject to DBS checks and based in Eastbourne. Car drivers and owners preferred as there will be extensive travel around East Sussex, much of which is rural and not served well by public transport.
