An opportunity has arisen for a Community Navigator in the Rural Rother area, to work in partnership with local GP’s to provide Social Prescribing support to promote peoples wellbeing and tackle ongoing social issues. This role is to provide holistic support and empower people to find the management tools to improve their Mental Health and Wellbeing.
To be successful in this role, you’ll need a good understanding of social issues that can impact on people’s lives, of existing support in their local communities and a genuine passion for supporting people to explore their needs.
Southdown are rapidly developing their Social Prescribing integration and this would be a good development opportunity for anyone interested in joining an experienced team working closely with the Primary Care Networks to provide short-term support to those facing social issues that are impacting in their Mental Health and Wellbeing.
